Paternity Leave Policy and Procedure

Human Resources Absence, Holidays and Sickness Region: | Reference: GPA04

This policy has been updated to provide more clarity to the Purpose section and also to provide additional information about how the various entitlements to family leave such as shared parental leave interact with paternity leave. This policy sets out the statutory minimum requirements that an employer must meet in the event an employee qualifies for paternity leave. Employers may choose to go beyond these requirements but must do at least what is set out in this policy. New documents have been added in the Forms section and references have been reviewed and updated.

Health and Safety Policy and Procedure

This is an overarching policy and will support staff to meet their legal health and safety obligations. It has been reviewed and changes made across the content to wording, amendment of sentences, paragraphs and section structures. No material change in effect. References have been reviewed and updated.

Serious Incident Notification Policy and Procedure

Health and Safety Health & Safety Region: | Reference: GHS08

This policy has been updated to reflect the updated guidance from the CQC about duty of candour and notifiable safety incidents, a link has been added to the underpinning knowledge section. Section 5.4 has been updated to include the 3 criteria which must be met for an incident to be a notifiable safety incident. The definition of harm and the definition of a notifiable safety incident have been added. A new significant events register is also available in the QCS Resource Centre. Existing references have been reviewed and updated.

Accident and Incident Reporting Policy and Procedure

This policy has been reviewed and updated to reflect the CQC updated guidance on meeting the duty of candour. Sections 4.3 and 5.4 have been updated and refer to completing a significant events register. A new definition has been added for the duty of candour. Significant Event Policy and Procedure

Administration Business Operations Region: | Reference: GAB30

This policy has been reviewed and updated to reflect the CQC updated guidance on meeting the duty of candour. Section 5.7 has been added to give further guidance about confidentiality. A new definition has been added for the duty of candour. Key facts for professionals, references and examples of outstanding practice have been updated. A new significant events register is available in the QCS resource centre. References have been reviewed and updated.

Patient Registration and Identity Check Policy and Procedure

Clinical Governance Risk Management and Safeguarding Region: | Reference: GCR17

This policy has been reviewed and section 5.9 updated for NHS providers to highlight that people must not be refused registration, particularly homeless people if they cannot provide ID. When registering with an NHS practice, homeless people must also be offered health screening and, if high risk, they must be prioritised for COVID-19 vaccination. A new reference has been added for improving GP registration among socially excluded groups and a link to Homelessness Training for GP Receptionists has been added to the Further Reading section. A new General Patient Registration form has also been added, particularly for non-NHS GP Practices to use. References have been reviewed and updated.

Medical Care Communication and Information Policy and Procedure

Quality Assurance Clinical Governance Quality Assurance Region: | Reference: GQG01

This policy has been reviewed with no significant changes to the content. A new Effective Consultation Listening and Observation Template has been added in the forms section for clinical staff to show evidence of open listening and caring skills. References have been reviewed and updated.

Staff Handbook Policy and Procedure

Human Resources Contract of Employment Region: | Reference: GPC00

This handbook directs staff to the relevant policies and sits over the top of the substantive policies on the QCS Management System. The policy has been updated to refer to GDPR and the Data Protection Act 2018, as well as making it clear it may be amended from time-to-time. References have been reviewed and updated.

Fire Safety Policy and Procedure

Health and Safety Buildings and Equipment Region: | Reference: GMR04

This policy will support staff with all aspects of fire safety at the premises of a service. It has been reviewed and amendments made to the Fire Risk Assessment procedure to include additional process steps. A new Fire Logbook Form has also been added to replace individual sheets. New references have been added and existing references have been reviewed and updated.

Single Use Equipment Policy and Procedure

Health and Safety Health & Safety Region: | Reference: GHS22

This policy has been updated and the form amended to record the type of single use items rather than each individual item. The content at 5.4 has also been updated to reflect this and a new procedure has been added at 5.5 to reiterate that single use and single person devices must not be re-used or shared and all staff must understand the risk to people who use services if they do not adhere to this. References have been reviewed and updated.
