This policy has been reviewed and updated to include a revised Employee Exit
Interview form and a new Employee Termination Report form. There are no significant
changes to the content and references have also been reviewed and updated.
This policy has been reviewed with no significant change to the content. However, the Induction Form has been revised and updated, and three new forms have been added to improve the checks carried out as part of the induction process for all new members of staff. Therefore a new Health and Safety Induction form, a new Staff Allergy Confirmation form, and a new Locum GP Induction form have been added to the Forms section.

Due to the change in societal norms, statistics indicate that it is expected that the uptake of Shared Parental Leave will soon become more prevalent. It is therefore important that relevant staff are well informed about this policy and are aware of the correct way in which the policy is to be implemented. This policy has been updated to include amendments to the structure and ordering of sections throughout the document. New definitions have been added and existing definitions amended for consistency across related policies. The procedural section has also been updated to include identifying responsible persons and immediate incident response and the remainder of the existing procedure simplified. Further reading has been added to include links to relevant policies and a link to Near Miss podcast episode. References have been reviewed and updated.
This policy has been reviewed and the content updated to include relevant notification processes, for example, the process for the level of investigation using Root Cause Analysis (RCA) at section 5.5, and an update to section 5.12 Health and Safety Executive to reflect the requirements for online reporting. New definitions and further reading have been added, and the Significant Event Form has been updated to include a section to record whether the incident was reportable. This policy has been reviewed and the content updated with the types of supervision in section 5.5. A new Clinical Supervision Record template has been added to the Forms section to enable supervisors to follow a structured process and document the outcomes of supervision meetings. This policy sets out the process for carrying out staff appraisals. It has been reviewed and updated to include probation reviews for new employees during the first six months of their employment. This is in the New Employees section of this policy at 5.4. A new Probation Review Form has also been added. References have been reviewed and updated.
This policy has been reviewed and the content updated to include information about breastfeeding and coronavirus and pregnancy. A new definition of ‘New or Expectant Mother’ has been added as well as underpinning knowledge about pregnant employees.
