Admission and Discharge Policy and Procedure

Care Planning Admission Risk Assessment Region: | Reference: SCA18

This policy has been fully updated to reflect the need to prevent unnecessary admissions to hospital and ensure effective, safe and timely discharges from hospital. Checklists are included to support clear communication between providers. The policy applies to transfer to or from hospital or respite care. The title has been changed from ‘Hospital Discharge Policy and Procedure’ to ‘Admission and Discharge Policy and Procedure’ to reflect the additional content and this policy replaces policy number SCA17.

A new policy for Induction and Onboarding. Achieving the right induction for staff is vital and the key to retention of staff. A positive experience can make all the difference. This policy identifies the procedures to follow to achieve a successful induction. Resources and further reading are available to enhance the core induction. This policy has a new reference code. The code was previously SPR07 and the title previously ‘Induction Policy and Procedure.’
