Participation Policy and Procedure

This policy details how individuals will be encouraged and supported to participate within the service, it discusses how the service will offer different means to enable people to be involved. The policy has been reviewed, with minor changes made to the procedure body, references checked and a Further Reading resource added to aid further knowledge and understanding.

Changes in Health State Policy and Procedure

This policy outlines how to approach changes in a person’s health state and the processes to follow should this happen. It has been reviewed with minor content changes, some Further Reading resources added and references updated to ensure they remain current.

Behaviour that Challenges Policy and Procedure

Care Planning Risk Assessment & Care Planning Region: | Reference: WCP05

The policy outlines the up-to-date theory and thinking about behaviour that challenges. The title of the policy has also been updated from ‘Challenging Behaviour’ to ‘Behaviour that Challenges’ Policy and Procedure. This policy is now inclusive of the de-escalation policy and procedure as it is recognised that this is a fundamental strategy for supporting behaviours that may challenge. The De-escalation Policy and Procedure has been archived. New resources added to support staff working with people that may present with behaviours that may challenge.

Care Communication and Information Policy and Procedure

Quality Assurance Care Management Quality Assurance Region: | Reference: WQC16

This policy highlights the need for good communication systems and the processes staff must follow to ensure this is embedded in the service and regularly reviewed. Policy merged with WAC02 Administration Communication and Action Policy and Procedure, which will now be archived. New procedural sections added as a result of this merge, and the form updated. References checked to ensure they remain current and more further reading added that staff should familiarise themselves with.

Restraint Policy and Procedure

Care Management Rights & Abuse Region: | Reference: WCR17

This policy outlines guidance in relation to restraint and restrictive practices, where
they are deemed necessary in line with current guidelines. It has been reviewed with
more information added in relation to restrictive practice and references checked and
updated.

Updated and expanded information in the new QCS format. More information given on communicating information and ensuring that Individuals are supported to communicate.

Policy updated to the new QCS format. Reviewed the focus from ‘meetings’ to the wider use of action planning including responses to CIW reports and updated the Form template.

Mobility Policy and Procedure

Care Management Care Practice Region: | Reference: WCC38

Policy reviewed and converted into the new QCS format. Emphasis placed on supporting independence and physical activity as well as mobilisation. This policy now dovetails with the Moving and Handling Policy and Procedure and the Falls Management Policy and Procedure. There is reference to the Social Services and Well-being (Wales) Act 2014 in relation to the promotion of wellbeing.

Medical Emergency Policy and Procedure

The medical emergency policy has been reviewed and converted to the new QCS policy format. The content has grown to include some further procedural standards in relation to taking a proactive approach to minimising the deterioration of an Individual before it becomes a medical emergency, as well as providing some clarity for actions to take when responding to a medical emergency.
