Refusal to Hold Records Policy and Procedure

It has been reviewed and updated and includes more explicit reference to the need to complete the form when it is the wish of the individual not to have records held at their home. In addition, reference to the new CIW lines of enquiry have been added to promote understanding and to provide supporting evidence.
