Whenever you have concerns that someone may be at risk of harm or abuse or has actually been harmed or abused, the first steps are to carry out an initial fact find.

At this stage you aren't carrying out an investigation merely, finding out, is someone in immediate danger, who is involved, when did the incident happen. This is to make sure you have the correct information to report and escalate the concern.
