How do I write a report that is robust to use at a disciplinary hearing for a referral of a client with behaviour increase?

Thank you for your question.

I believe that it is essential that when preparing a report for such a purpose that the report needs to be as accurate as possible and it also of course needs to be factual.

What I have always tried to do when preparing a report is to keep everything in chronological order, this order by date is then easy for everyone to read and will of course be factual.

Any conclusions that you come to or personal opinions should come at the end.

Providing that you have all the facts recorded, then suddenly preparing a report becomes easier.

It is the systematic recording of information in the past that is the most helpful when having to complete a report.
