The simple answer here is no. Every service and every provider differ. Electronic assessments, care plans and off the shelf tools would still need to be reviewed and tweaked by you to ensure they are what you can realistically offer, support and reflect what your service’s regulated activity is and what services, specialities you provide.

You may find the “What to expect during assessment and care planning” guidance produced by NICE a good starting point to ensure what you are using covers all of the points highlighted.

You can find the guide here: https://www.nice.org.uk/about/nice-communities/social-care/quick-guides/what-to-expect-during-assessment-and-care-planning

CQC Regulation 9: Person Centred Care also defines your assessment process and some of the things that as a provider you must ensure.

You can find the regulation here: https://www.cqc.org.uk/guidance-providers/regulations-enforcement/regulation-9-person-centred-care#full-regulation

SCIE also have some great resources that you might find useful on integrated care, joint needs assessment and care planning: https://www.scie.org.uk/integrated-care/research-practice/activities/joint-needs-assessment-care-planning
