GP - CQC Registration
Serious Incident Notification Policy and Procedure
This policy has been reviewed and the content updated to include relevant notification processes, for example, the process for the level of investigation using Root Cause Analysis (RCA) at section 5.5, and an update to section 5.12 Health and Safety Executive to reflect the requirements for online reporting. New definitions and further reading have been added, and the Significant Event Form has been updated to include a section to record whether the incident was reportable. References have been reviewed and updated.
Quality Assurance and Good Governance Policy and Procedure
This policy has been reviewed and the title updated to Quality Assurance and Good Governance Policy and Procedure, previously it was Quality Assurance Policy and Procedure. This is to reflect the additional content, definitions and further reading in relation to Regulation 17: Good governance, which requires providers to monitor and drive improvement in the quality and safety of the services provided, including the quality of the experience for people using the service. Existing references have been checked and updated.

This policy has been reviewed with no significant changes to the content. Further reading has been added including links to relevant policies. References have been reviewed and updated.
Statement of Purpose Policy and Procedure
This policy has been reviewed with minor changes to the content to give specific examples in some sections. New references and further reading have been added and existing references have been reviewed and updated.
Medicines Policy and Procedure
This policy has been updated to reflect the latest CQC guidance on non-medical prescribing. Additional content including a new policy statement, procedure, references and further reading has been added, as well as actions for the Practice to conduct staff risk assessments to ensure that staff are appropriately qualified and registered and take appropriate steps to assure safety. Existing references have also been reviewed and updated.
Staff Requirements and Involvement Policy and Procedure
This policy has been reviewed with minor changes to the content. Additional references and further reading have been added, including links to other relevant policies. Existing references have been reviewed and updated.
Health and Safety Training Policy and Procedure
Policy has been updated to include details of health and safety training topics. Two new forms have been added, one to record staff feedback following health and safety training, and one is a record of health and safety training undertaken by each member of staff. References have been reviewed and updated.

Consultation and Treatment or Care Pathways Policy and Procedure
Policy has been updated to include further reading from the CQC about looking at the quality of care pathways and co-ordinated care, and the Integrated Care Systems (ICSs) due to be established nationally by April 2021 to provide better and more joined-up care for Patients. References have been reviewed and updated.
Practice Merger and Closure Policy and Procedure
This policy has been reviewed and its title changed from ‘Practice Closure Policy and Procedure’ to ‘Practice Merger and Closure Policy and Procedure’ to include the considerations and the process for a Practice merger, as well as closure. Additional content, definitions and further reading have been added. References have been reviewed and updated.
