Diabetes Policy and Procedure

Care Management Care Practice Region: | Reference: ICC11

This policy details how the organisation will support service users who are at risk of or present with Diabetes. Based on recommendations from Diabetes Ireland, HSE and National Guidelines (including UK NICE) it sets out how to assess, support and manage risk and provides guidance and tools for staff to use to aid understanding and a consistent and safe response. The policy has been reviewed and further content added to support staff practice. This includes the following:

Procedure for calibration of blood glucose machines
Increased content for managing hypoglycaemia
Weight management
Insulin management 
Supporting service users to self-manage
Managing Diabetes during COVID-19 links
